NPM ships SQLite built in — no external database required by default. Postgres and MySQL/MariaDB are optional alternate backends; picking one disables SQLite (NPM's own docs: 'DB_MYSQL_* environment variables will take precedent over DB_SQLITE_* variables... you will not be able to use SQLite'). Use at most one alternate backend, never several.
